Information about the baby
You will need to bring a certificate from the doctor or midwife to the Register Office. The registrar will also need:
- the date and place of the stillbirth
- the full names and surname if the baby is to be given a name
- the sex of the baby
Information about the mother
The registrar will need the following information about the mother:
- full names and surname, including maiden name if applicable
- date and place of birth
- occupation or, if not currently employed, any previous occupation
- usual address
- date of marriage, if married to the baby’s father
- number of previous children

The registrar will need the following information about the father:
- full names and surname
- date and place of birth
