When registering the birth, you should know:
- date and place of baby's birth
- for multiple births such as twins and triplets, the time of each baby’s birth
- forenames, surname and sex of the baby
- full names of parents and any other names they are known by or have previously been known by
- parents’ dates and place of birth
- parents’ jobs
- parents’ date of marriage (if applicable)
- mother’s maiden surname
What you should take
You should take at least one form of identification when you go to the register office. You can use:
- passport
- birth certificate
- deed poll
- driving licence
- proof of address (for example, a utility bill)
- Council Tax bill
- marriage or civil partnership certificate
You will also need to show the baby's NHS number wich can be found on the:
- discharge notes form the hospital or midwife
- red book
