- prevention and early support
- sensory and equipment services
- hospital discharge pathways
- locality teams for the North and South of the borough
- providing local teams for each of the five neighbourhoods to enable seamless care with health and voluntary, community, and social enterprise (VCSE)
- specialist teams for learning disability, autism, and mental health
Our focus
Our internal provider services focus on helping people enjoy the best everyday life possible, using a truly personalised approach. We promote dignity, privacy, and informed choice, creating chances for people to grow in areas such as communication, mobility, independence and decision‑making which includes:
- support to live well - providing tailored support for people with learning disabilities, autism and complex needs, including access to supported living across eight sites and a nine‑bed respite service offering planned and emergency stays for around 60 families
- support to age well - delivering reablement, extra care and daily support for older people, those living with dementia and people with physical or learning disabilities, helping individuals regain independence, stay well at home and engage in community life
- day opportunities - offering community‑based activities for people with learning disabilities, autism, complex health needs and older adults, providing structured programmes, specialist groups and inclusive opportunities to reduce isolation and build confidence
