Report provider quality concerns

How to report a concern with a care provider if you are a professional.

We have a process in place for professionals to report any quality concerns that you may have with providers so that these can be formally monitored. 

The information provided will be used to assist us in managing and prioritising our performance monitoring and service provision.

To receive a copy of the provider quality monitoring concerns form:

Email: hclcqteam@luton.gov.uk 

Once received the Provider Quality Assurance Team will acknowledge receipt of your concern and will indicate what future action they will take.

Before completing concerns form, consider the following. This will ensure that you are using the correct and most appropriate process to raise your concerns. 

Is this a complaint? 

If this matter is an issue which relates to the dissatisfaction of services an individual is receiving:

Is this a safeguarding issue? 

If this matter relates to a potential safeguarding issue, complete the safeguarding alerts form and email securely to adultsafeguarding@luton.gov.uk.
