This is a service designed to support families with a child, aged 0-5 years old, who has a complex care needs requiring services from more than one of the service providers i.e. Social Care, Health, Education
The service is a key worker service giving a family one named person, a care coordinator, who can give help and support and point them to services they may require.
We are able to offer a designated care coordinator to support families from the South Asian Community.
(The serivce is provided jointly by Luton Borough Council’s ISCAN and Health).
