How can I get care and support?

First we must work out if you are eligible for council-funded care If you are not eligible there is still a lot of help available in Luton.

'Care and support’ is the term used to describe the help some adults need when they are unable to complete day-to-day tasks because of a physical disability, a learning disability, a long-term health condition, frailty due to old age, mental ill-health or substance abuse.

Care and support is also available to carers who care for an adult friend or family member and this has an impact on their own wellbeing.Care and support can include:

  • support, home adaptations or home care to help you to remain independent in your own home
  • help to access a suitable care home, when living at home is no longer an option
  • temporary support, like reablement or rehabilitation after an accident, illness or stay in hospital

Step 1 - Contact us to tell us about your individual support needs

Find out more about an assessment of care needs.

There's now a quicker and easier way to find out whether you, or a loved one, are eligible for care and support. Our online self-assessment will ask you about the activities you might need support with and will let you know whether you are eligible. There’s also an online financial assessment and a carers assessment.

Step 2 - Find out if you are eligible for care and support

First we must work out if you are eligible for council-funded care. Once you've completed your online self-assessment

You'll get a call from a social worker who will talk to you in more detail about the answers you gave on your assessment.

If you're eligible for support the social worker will find out more about the type of care you require.

Step 3 - Your personal budget

We'll give you an estimated budget to spend on the type of care and support to fit your assessed needs.

Step 4 - Your care and support plan

You create a care and support plan which will show how you've chosen to spend your personal budget.

Step 5 - Your care and support can now be put in place

Once you and your social worker agree on your care and support plan, your services can be put into place.

Step 6 - Your financial assessment

We'll look closely at your finances to see if you have to contribute towards the cost of your care and support.

Step 7 - Regular reviews

Your progress will be reviewed regularly and any necessary changes made. You can request a review at any time.
